As you prepare to enroll:

Consider your benefit coverage needs for the upcoming year. For example, is your family financially protected if you can't work due to an accident or illness?

  • Consider other available coverage
  • Gather information you'll need. If you are covering dependents, you will need their dates of birth and Social Security numbers. In addition, you may need to provide documentation verifying their eligibility - such as a marriage license or birth certificate.

Open enrollment has begun! This is your once-a-year opportunity to review your benefits, make changes, and ensure you have the coverage that best fits your needs. ELIGIBILITY

Who's Eligible?

Team Members:

Team members working 30 hours or more per week are eligible for all company benefits after meeting each plan's eligibility requirements. You are first eligible for benefits on the 1st day of the month following 30 days of continuous service.

Dependents:

You may choose to cover eligible dependents, including:

  • Your spouse
  • Child(ren) under the age 26, regardless of marital status

Enrolling dependents? Have these items ready:

When you add dependents to your coverage, you must provide the following information:

  • Legal name
  • Date of birth
  • Social Security number
  • Supporting documentation, such as marriage certificate, birth certificate, adoption papers, and tax documents.

If you do not provide the required information, your dependents may be dropped from coverage.

Enrollment Periods:

New Team Members:

As a new team member, you are first eligible for benefits on the 1st day of the month following 30 days of continuous service.

Open Enrollment:

As a benefits eligible team member, you have the opportunity to enroll in or make changes to your benefit plans during our annual open enrollment period. Open enrollment is usually held in October with benefit elections effective January 1.

Changes:

Making changes to your benefits

Generally, you may only make or change your benefit elections as a new hire or during the annual Open Enrollment period. However, you may change your benefit elections during the year if you experience an event such as:

  • Change in the number of your dependents (through birth, adoption, or if a child is no longer an eligible dependent)
  • Change in a spouse's employment status (resulting in a loss or gain of coverage)
  • Change in legal marital status (marriage, divorce, or legal separation)
  • Change in employment status: from full-time to part-time or part-time to full-time, resulting in a gain or loss of coverage
  • Gain or loss of coverage through the Marketplace
  • Gain or loss of Medicare or Medicaid
  • A court order to provide health coverage for your eligible dependent (QMSCO)

IMPORTANT:

  • If you need to make a change before the next Open Enrollment period due to a change in status, you must submit the required documentation within 31 days of all Qualified Events.
  • Depending on the type of event, you may need to provide proof of the event, such as a marriage license
  • If you do not make the changes within 31 days of the qualified event, you will have to wait until the next Open Enrollment period to make changes (unless you experience another qualified life event).
