LIFE & DISABILITY INSURANCE

Life & Disability
Life, AD&D and Disability insurance can fill a number of financial gaps due to a temporary or permanent reduction of income. Consider what your family would need to cover day-to-day living expenses and medical bills during a pregnancy or illness related disability leave, or how you would manage large expenses (rent or mortgage, children's education, debt, etc.) after the death of a spouse.

Protecting your inner circle
We know you want more options, so Voluntary Life Insurance allows you to purchase life insurance to protect your family's financial security. Coverage is available for your spouse and/or child(ren) if you purchase coverage for yourself.
What is Evidence of Insurability (EOI)?
Evidence of Insurability is additional information about your health status in order to qualify for the requested amount of coverage. If a team member elects a coverage that requires an EOI, Prudential will mail you the EOI to complete after open enrollment. If a team member fails to submit the EOI to Prudential, the team member's voluntary life insurances will be canceled.
What is Guaranteed Issue?
If you select voluntary coverage above a certain limit (the "guaranteed issue") or after your initial eligibility, you will need to provide additional information about your health status in order to qualify for the requested amount of coverage.
What is a Beneficiary?
A beneficiary is the person or legal entity who receives the life insurance benefit in the event of your death. You can name as many beneficiaries as you like, but you’ll need to designate a percentage of the benefit for each. If you select more than one beneficiary, be sure the total percentage adds up to 100%.
Voluntary Life Monthly Premiums
Rates are based on $1,000 selection basis.
